Youth Registration

Please download the Welcome Package for club details and the Application form and the waiver of Liability below to get started and you can either mail them to the address listed below with a cheque or bring it into the club on one of of Registration days to get started! *Forms will be available at the club on the registration days.

Additionally, it is deemed that the guardian of youth members agree to the below when submitting an application to the club.

*I/we hereby give permission to the coaches of Prescott Curling Centre Inc. to take the youth member listed in this form to the hospital if need be and I/we also give permission to have the child treated by a physician in the parent/guardian’s absence. Further, I/we do hereby waive, release and forever discharge Prescott Curling Centre Inc., its staff, officers, agents, representatives, employees, members and their successors and assigns from any and all claims for bodily injury or property damage whether said accident, injury or loss is due to negligence or not. By checking this box on this form, I/we hereby release Prescott Curling Centre Inc. from any and all possible injuries incurred with respect to the above child while on Prescott Curling Centre premises or attending out-of-town events.

Rowan’s Law:

This law was enacted on July 1, 2019, and concerns concussions. Everyone who participates in curling or any sport who is 26 or under must read the information presented and sign a confirmation to that effect. This confirmation will be kept on file and this procedure followed every year. There are three booklets of information:

10 Years and under

Bantam - 11 to 14 years of age

Junior - 15 years of age to 20

You MUST download, read and sign this waiver of liability, Rowan’s Law confirmation form and the Media release form (on application form) and then bring or send it to the club.
